The Golden Dental Centre is committed to maintaining the confidentiality, security, and accuracy of the Personal Information provided online through their website. Website Users may use the Golden Dental Website without giving Personal information but basic Personal Information is required to submit a contact submission through the Website. More detailed Personal Information such as but not limited to date of birth and medical conditions are required for a patient to complete the new patient intake form on the Website. 

This Privacy Policy describes how the Golden Dental Centre collects, stores, uses, and shares all Personal Information that is received by email, phone, in-person or online. 

This Privacy Policy also describes how the Golden Dental Centre monitors website traffic and their Website’s cookie usage.

The words of which the initial letter is capitalized have meanings defined under the following conditions. The following definitions shall have the same meaning regardless of whether they appear singular or plural.

Personal Information refers to any information such as, but not limited to, personal name, email address, and phone number, that is identifiable to an individual

The Golden Dental Centre refers to The Golden Dental Centre and its affiliates, both internal and external.

User refers to an individual using the Golden Dental Website

Cookies refer to small files that are placed on the User's computer, mobile device or any other device by a website, containing the details of the User’s browsing history on that website among its many uses.

PERSONAL INFORMATION

Personal Information

The Golden Dental Centre collects and maintains different types of Personal Information about a User with whom it interacts (such as those who seek to be, are, or were patients or suppliers of the Golden Dental Centre), including:

  • identification and contact information such as a patient’s name, home address, telephone, email address, date of birth, health information, marital and dependent status, videos, photographs,  and emergency contact information;

  • benefit information such as but not limited to a User’s dental insurance company, policy number, identification number and plan breakdowns; and

  • other information necessary for Uthrive’s business purposes, which may be voluntarily disclosed or collected

Use & Disclosure

The Golden Dental Centre respects the privacy of its employees, suppliers, patients and clients and ensures that the Personal Information collected remains secure and protected. Only employees of the Golden Dental Centre who need the information to perform a specific job have access to Personal Information.

The Golden Dental Centre does not automatically gather any Personal Information from a User, such as a User’s name, phone number, email, or address. This information is only obtained if Users supply it voluntarily, usually by contacting the Golden Dental Centre via email, completing new patient intake or insurance information form found on the Website. By choosing to provide the Golden Dental Centre with Personal Information, Users are consenting to its use in accordance with the principles outlined in this Privacy Policy and as outlined at the time a User is asked to provide any Personal Information.

There are times when Personal Information must be shared with an interested third party, such as but not limited to a dental specialist. In such circumstances, The Golden Dental Centre will not release any Personal Information collected through the Website without first advising of its intended use and obtaining permission to do so from the affected User.

Protection of Personal Information

The Golden Dental Centre has secure databases where Personal Information is stored and may also store Personal Information in paper document files in their offices. Any Personal Information a User does provide is managed according to British Columbia’s Freedom of Information and Protection of Privacy Act (FOIPPA). This means that, at the point of collection, Users will be informed that their Personal Information is being collected, the purpose for which it is being collected and that Users have a right to access the information.

Email transmission can be accessed by other Internet users. If a User wants to keep certain information confidential they may contact the Golden Dental Centre by telephone, in person, or via mail.

The Golden Denal Centre’s Privacy Policy is designed to meet or exceed the requirements of Canadian Federal and Provincial privacy laws. The Golden Dental Centre constantly self-evaluates practices and procedures to maintain the highest standard in this regard.

The Website will use a User’s Personal Information to contact them for support purposes. If Users wish to be removed from the contact database, please email the Golden Dental Centre at info@goldendental.ca

Retention of Personal Information
The Website has set mandatory retention time limits based on Provincial Employment and Labour Standards and third-party suppliers are instructed to follow the same guidelines. Appropriate safeguards relevant to the sensitivity of the information will be employed.

Updating Personal Information
The Golden Dental Centre will make every effort to ensure Personal Information remains current and is updated internally and with third-party suppliers in a timely manner. Users are expected to update the Website with any changes to their Personal Information. The information will only be updated when required for the intended, previously described purposes.

Accessing Personal Information
The Golden Dental Centre will release User Personal Information to the User upon request, along with the use and disclosure of any Personal Information. Specific requests for information can be directed to the Privacy Officer via the info@goldendental.ca email address. The information will be released within 30 days of receipt of the request. Should the Website not be able to comply with the request, a justifiable reason will be given to the requestor.

The Website considers the following justifiable reasons to refuse access to personal information.

  • Solicitor-client privilege

  • Litigation privilege

  • Security and ongoing fraud investigations

  • Proprietary or trade secret information

  • Sensitive medical information that could be obtained through other means

  • Requests that would prove prohibitively costly to provide

  • Requests that could reasonably be expected to threaten the life or security of another individual

This list is not exhaustive and other reasons may be considered justifiable.

MONITORING WEBSITE TRAFFIC

Monitoring Website Traffic

Like most web servers on the Internet, the Website’s servers maintain access logs that record web browser activity. These logs associate an Internet Protocol (IP) address with each request for content from the servers.

When Users visit the Website, the web server automatically collects a limited amount of standard information essential to the operation and evaluation of the Website. This information includes:

  • the page from which the User arrived,

  • the date and time of the User page request,

  • the IP address the User’s computer is using to receive information,

  • the type and version of the User’s browser, and

  • the name and size of the file the User request

This information helps to assess the Golden Dental Centre’s information services and is used only for this purpose. It is collected in compliance with section 33 of the Freedom of Information and Protection of Privacy Act (FOIPPA).

This information is collected to help make the Website more useful to visitors, to learn about the number of visitors to the Website, and to learn about the types of technology visitors use. The Golden Dental Centre does not track information about Users and their visits. The Golden Dental Centre will not identify Users or their browsing activities, except as required by a law of British Columbia or Canada or if compelled to produce this information for a legal proceeding.

For security purposes and to ensure that this service remains available to all Users, this computer system employs software programs to monitor network traffic to identify unauthorized attempts to upload or change information, or otherwise cause damage.

Except for authorized law enforcement investigations, no other attempts are made to identify individual users or their usage habits.

Website Cookie Usage

The Golden Dental Centre has web servers that can use “cookie” technology to keep track of the web browsers with which they are communicating.

During a cookie transaction, a web server will hand a web browser some data prescribed by the website’s developers. The web browser will return that data with its next request for content from the website. Further, the Website does not match for its records any session-identifying cookie information with the Internet Protocol (IP) addresses described above in the information in section IV. Monitoring Website Traffic.

The Golden Dental Centre is committed to the security of Personal Information it may collect online or offline. The Golden Dental Centre has taken reasonable precautions to protect this Personal Information from loss, misuse or alteration. Any third parties who may have roles in managing such information on behalf of the Golden Dental Centre are also committed to the same principles.

THIRD-PARTY LINKS

The Website includes links to web pages or sites operated by third parties. The Golden Dental Centre endeavours to make it clear that certain links will take Users away from the Website and to destinations over which the Golden Dental Centre has no control.

The Golden Dental Centre’s Website Privacy Policy statement does not apply to the sites of third parties to which there are links. Instead, the Golden Dental Centre encourages Users to review the policies of these sites themselves upon their arrival.

Users have the right to file a challenge about this Website’s compliance with Privacy legislation.
