Step 1

If you haven’t already, the first thing to do is to contact our office to book an appointment. Our administrative team will be happy to help you book the right appointment for your needs. Feel free to call or email us to schedule your appointment.

Step 2

New Patient Information

After booking your first appointment, the next thing to do is to fill out our intake form. Make sure to press “Submit” and a confirmation that the form has been submitted will come up. Filling out this form beforehand allows your appointment to be focused on treatment, not paperwork.

Insurance Information Form

If you have insurance, we ask that you fill out our insurance information form.

Having trouble with this form? You can email us a photo or screenshot to info@goldendental.ca of the insurance numbers, so we can set up the insurance on your chart.

NOTE: This form needs to be signed by the insurance policyholder.

X-ray Release

Finally, prior to your appointment have your previous office email us any recent x-rays to info@goldendental.ca

Step 3

What To Expect

On your first visit, you will be greeted by our lovely administrative team. They may ask a couple of questions and are happy to answer any of yours. When the room is ready, one of our dental hygienists or dental assistants will take you back to one of our stylish new treatment rooms. They will start the appointment by going over your medical history, checking to see if taking new x-rays are recommended and starting to chart out any existing treatment you have had in the past. They may also do an oral health scan so you can better see what we are seeing. Your dentists will come by andmexamine your mouth. They will then discuss their findings and create a treatment plan if needed, and answer any questions you may have.
